Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our expanding team. The ideal candidate will possess a proactive attitude, outstanding interpersonal skills, and a valid state LPN license. In this role, you will provide essential medical support to our clients while ensuring their safety and well-being.

Responsibilities:

  • Collaborate with the Registered Nurse (RN) and physician to plan and coordinate comprehensive patient care, adhering to the nursing care plan for each client.
  • Accurately document and report on the patient’s condition and care, identifying any signs or symptoms indicative of potential changes.
  • Prepare clinical progress notes and summaries as required.
  • Assist patients with activities of daily living while instructing them in effective self-care techniques.
  • Maintain a safe environment for patients at all times.
  • Provide assistance to the physician and Registered Nurse during complex nursing situations.
  • Implement universal precautions during all aspects of patient care.
  • Follow emergency protocols in the event of critical incidents, including accidents, injuries, or significant changes in patient conditions.

Qualifications:

  • Minimum of one year of experience as an LPN in a home care setting.
  • Active state LPN license.
  • Graduate from an accredited practical nursing program.
  • Proof of current CPR certification.
  • Flexibility in scheduling and willingness to travel locally.
  • Outstanding interpersonal skills.
  • Valid driver’s license and reliable transportation.
