Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to become a vital member of our expanding team. The ideal candidate will possess a positive attitude, exceptional interpersonal skills, and a current state LPN license. In this pivotal role, you will be responsible for delivering medical support to our clients while maintaining a safe and nurturing environment.

Responsibilities:

• Collaborate in the planning and coordination of comprehensive patient care alongside the Registered Nurse (RN) and the physician’s plan of care, adhering to the individualized nursing care plans for each patient.
• Accurately report and document the patient’s condition and care, noting any signs or symptoms that may indicate a change.
• Prepare clinical progress notes and summaries to provide a clear narrative of patient status.
• Support patients in their activities of daily living while imparting appropriate self-care techniques.
• Ensure and uphold a safe environment for patients at all times.
• Assist the physician and Registered Nurse in managing complex nursing scenarios.
• Adhere to universal precautions in all aspects of patient care to maintain safety standards.
• Implement emergency procedures in response to any catastrophic event or significant change in a patient’s condition.

Qualifications:

• A minimum of one year of experience as an LPN in a home care setting.
• Current state licensure as a Licensed Practical Nurse.
• Graduate of an accredited practical nursing program.
• Valid proof of current CPR certification.
• Flexibility to work varied schedules.
• Excellent interpersonal communication skills.
• A valid driver’s license and reliable transportation.
• Willingness to travel locally as needed.
