Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our expanding team. The ideal candidate will exhibit a proactive attitude, possess exceptional interpersonal skills, and hold a current state LPN license. In this position, you will provide medical support to our clients while ensuring their environment remains safe and conducive to care.

Responsibilities:

  • Collaborate with the Registered Nurse and physician to participate in the planning and coordination of comprehensive patient care, adhering to the established nursing care plan for each individual.
  • Accurately document and report patient conditions and care, noting any signs and symptoms that may indicate changes in health status.
  • Prepare clinical/progress notes and summaries as required.
  • Assist patients with activities of daily living while educating them on appropriate self-care techniques.
  • Ensure a safe environment for patients at all times.
  • Support the physician and Registered Nurse in complex nursing situations as needed.
  • Adhere to universal precautions during all aspects of patient care.
  • Execute emergency procedures in the event of any critical incidents, such as accidents, injuries, or significant changes in a patient’s condition.

Qualifications:

  • A minimum of one year of experience as an LPN in a home care setting.
  • Current state licensure as an LPN.
  • Graduation from an accredited practical nursing program.
  • Proof of current CPR certification.
  • Ability to work a flexible schedule and travel locally as required.
  • Exceptional interpersonal skills.
  • Valid driver’s license and reliable transportation.
