Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to become an integral part of our expanding team. The ideal candidate will possess a proactive attitude, exceptional interpersonal skills, and a current state LPN license. In this role, you will provide vital medical support to our clients while ensuring their safety and well-being.

Responsibilities:

  • Collaborate with the Registered Nurse (RN) and physician to plan and coordinate comprehensive patient care, adhering to each patient’s nursing care plan.
  • Accurately document and report the patient’s condition and care, noting any signs or symptoms that may suggest changes in their health status.
  • Prepare clinical progress notes and summaries as required.
  • Assist patients with daily living activities while educating them on appropriate self-care techniques.
  • Ensure a safe environment for patients at all times.
  • Support the physician and Registered Nurse in complex nursing scenarios.
  • Adhere to universal precautions during all aspects of patient care.
  • Execute emergency procedures in response to any critical incidents such as accidents, injuries, or significant changes in the patient’s condition.

Qualifications:

  • Minimum of 1 year of experience as an LPN in a home care setting.
  • Valid state license as a Licensed Practical Nurse.
  • Graduation from an accredited practical nursing program.
  • Current CPR certification.
  • Flexibility in scheduling to meet patient needs.
  • Exceptional interpersonal skills.
  • A valid driver’s license and reliable transportation.
  • Willingness to travel locally as required.
