Job description:
We are seeking a compassionate Licensed Practical Nurse (LPN) to join our expanding team. The ideal candidate will possess a positive, can-do attitude, demonstrate exceptional interpersonal skills, and hold a current state LPN license. In this role, you will provide essential medical support to our clients while maintaining a safe and supportive environment.
Responsibilities:
- Collaborate with the Registered Nurse (RN) and physician in planning and coordinating comprehensive patient care, ensuring adherence to the nursing care plan for each individual.
- Accurately assess, report, and document the patient’s condition and care, including any signs and symptoms indicative of changes in health status.
- Prepare detailed clinical progress notes and summaries.
- Assist patients with daily living activities, promoting and teaching appropriate self-care techniques.
- Ensure the maintenance of a safe environment for every patient.
- Support the physician and Registered Nurse in complex nursing situations.
- Adhere to universal precautions in all aspects of patient care.
- Implement emergency procedures in the event of catastrophic incidents, injuries, or significant changes in a patient’s condition.
Qualifications:
- A minimum of one year of experience as an LPN in a home care setting.
- Valid state license as an LPN.
- Graduate of an accredited practical nursing program.
- Proof of current CPR certification.
- Flexibility in working hours and scheduling.
- Strong interpersonal skills.
- Valid driver’s license and reliable transportation.
- Ability to travel locally.
