Responsibilities:
The Licensed Practical Nurse (LPN) is responsible for planning, coordinating, providing, and managing nursing care services and health education for nursing home guests. Key responsibilities include supervising the care provided by Certified Nursing Assistants (CNA)/State-Tested Nursing Assistants (STNA) and other team members, ensuring safe and accurate medication administration, assessing guest health and reporting changes to the physician while promptly implementing new orders, and selecting appropriate nursing interventions to stabilize guest conditions and prevent complications. Additionally, you will contribute to the guest assessment (MDS/CAA’s) and assist in the development of individualized care plans.
Qualifications:
– Preferred two years of experience in a sub-acute skilled nursing and rehabilitation setting
– Experience working with tracheostomies, laryngectomies, IV therapy, central lines, chest ports, TPN, tube feeding, g-tubes, wound care, ostomies, and LVADs is preferred
– Valid Licensed Practical Nurse (LPN) licensure in the state
– CPR certification
