We are currently looking for a Licensed Practical Nurse (LPN) to join our team at the OhioHealth Neuro Transitional Center in Dublin, OH. This is an exceptional opportunity to make a significant impact in the lives of individuals undergoing rehabilitation for acquired brain injuries or spinal cord injuries. Our facility boasts a 12-bed capacity, designed to provide intensive rehabilitation for patients who are not yet ready to return home following discharge from an inpatient rehabilitation hospital. Impressively, 93% of our patients achieve the ability to return home after their stay with us, allowing Ohio residents to access this vital level of care without needing to leave the state.

As a Licensed Practical Nurse (LPN) / Neuro Rehab Coach Supervisor, you will oversee the Neuro Rehabilitation Coaches (NRC) who provide direct patient care, while being an integral member of the neuro transitional rehabilitation team. Your role will involve assisting the NRCs with treatment plans, ensuring the well-being and safety of all patients, and offering support to visitors while maintaining smooth operations during your shift. You will also play a crucial role in educating patients, families, and colleagues.

Key Responsibilities:
– Supervise staff during your assigned shift, providing guidance as necessary and communicating any staff-related concerns to the Neuro Transitional Center Leadership or RN/Administrator On Call.
– Collaborate with the transitional team to ensure that patient care plans are followed and assist in rehabilitation activities, facilitating patient engagement and goal achievement across various environments.
– Work with NTC leadership to maintain appropriate staffing levels in accordance with guidelines and patient needs, escalating any issues to the supervisor or Administrator on Call.
– Support patients with meal planning, food preparation, and grocery shopping, as well as manage inventory and overall housekeeping responsibilities as assigned.
– Coordinate and provide transportation for patients, ensuring adherence to related policies concerning vehicle maintenance and logs.
– Participate in scheduled training and meetings, as necessary.
– Ensure accurate documentation and supervise NRCs in compliance with Neuro Transitional Rehab policy.
– Carry out all delegated nursing tasks, including vital signs, gastric tube management, diabetic care, medication administration, and trach care.
– Administer medications for a maximum of 12 patients.

Minimum Qualifications:
– Active Licensed Practical Nurse (LPN) designation.
– Valid State Driver’s License in good standing.
– Completion of Certified BLS; new hires should refer to CPR Requirement Guidelines.

Preferred Qualifications:
– Two years of experience in a residential care facility or rehabilitation environment, particularly with brain injury patients.
– Previous experience as a medication aide.
– Demonstrated ability to delegate and effectively solve problems.
