Job description:
We are currently looking for a Neuro Rehab Coach Supervisor to join our newly established OhioHealth – Neuro Transitional Center located in Dublin, OH. This position offers an exceptional opportunity to make a significant impact in the lives of individuals undergoing intensive rehabilitation for acquired brain injuries or spinal cord injuries, who are not yet able to safely return home after an inpatient rehabilitation hospital stay. Notably, 93% of our patients successfully transition back to their homes at the time of discharge from the Neuro Transitional Center, providing an invaluable service to residents of Ohio.

As a Neuro Rehab Coach Supervisor, you will oversee the Neuro Rehabilitation Coaches (NRC) who deliver direct patient care, ensuring their efforts align with treatment plans while safeguarding the wellbeing and safety of all patients. You will also play a vital educational role for patients, their families, and fellow team members.

Key Responsibilities Include:
– Supervising staff during your assigned shift, offering guidance when necessary and effectively communicating any concerns to the Neuro Transitional Center Leadership or RN/Administrator On Call.
– Supporting and supervising the NRCs in implementing patient care plans in collaboration with other transitional team members while promoting patient engagement and educational efforts.
– Coordinating with leadership to maintain appropriate staffing levels in accordance with patient needs and staffing guidelines; addressing any issues with the NTC supervisor or Administrator on Call.
– Assisting patients with meal planning, food preparation, and grocery shopping, while managing grocery and inventory needs, as well as maintaining suite management and housekeeping tasks as assigned.
– Coordinating patient transportation, ensuring adherence to policies regarding vehicle maintenance, logs, and fueling.
– Participating in scheduled training sessions and meetings as required, ensuring continuous professional development.
– Completing and overseeing documentation to meet Neuro Transitional Rehab policy requirements.
– Performing nurse delegation tasks as assigned, including vital signs monitoring, gastric tube management, and administration of medications for a maximum of 12 patients.

Qualifications:
Minimum Qualifications:
– A valid Licensed Practical Nurse (LPN) credential.
– Current State Driver’s License with a good standing.
– Certification in Basic Life Support (BLS); new hires should consult the CPR Requirement Guidelines for New Hires.

Preferred Experience:
– A minimum of two years in a residential care facility or rehabilitation setting, particularly with brain injury populations.
– Experience as a medication aide.
– Proven ability to delegate tasks effectively and solve problems efficiently.
