Role Overview:

In this role, you will assess residents’ needs comprehensively and develop personalized care plans tailored to those assessments. Key Responsibilities:
- Oversee the Resident Assessment Instrument (RAI) process to evaluate residents’ individualized needs and formulate unique care plans based on your assessments.
- Foster cross-functional communication to ensure adherence to care plans.
- Advocate for appropriate reimbursement for resident care services.
- Stay informed about current federal, state, and local standards, guidelines, and regulations.
Qualifications:
- A current, valid state RN license is required.
- A minimum of 2 years of MDS (resident assessment) experience.
- Prior experience in long-term care is mandatory.
