As a Clare Bridge Program Assistant, you will assist the Clare Bridge Program Manager or Coordinator in planning and executing a calendar of person-centered, dementia-friendly programs tailored to the specific needs of our residents. In alignment with Brookdale’s Resident and Family Engagement philosophy, you will facilitate meaningful and purposeful engagement within the dementia care community, thereby enhancing residents’ quality of life. Your role will involve recognizing individual capabilities and preferences while fostering independence and honoring each resident’s historical routines and interests.

Essential Functions:

  • Support the delivery of a person-centered program that caters to the unique needs and preferences of residents, engaging them across the seven domains of well-being: purposeful, intellectual, social, spiritual, emotional, and physical. This includes organizing sensory-focused small groups for residents who may struggle in larger settings.
  • Collaborate with the Program Manager or Coordinator in reviewing resident service plans and completing necessary program documentation in accordance with Brookdale standards and state regulations.
  • Ensure compliance with state, federal, and organizational regulations regarding dementia care programs while upholding Brookdale’s mission, policies, and standards.
  • Provide guidance and resources to associates concerning the significance of programming and engagement to enhance residents’ quality of life.
  • Coordinate and engage in community events, including family socials and marketing initiatives.
  • Maintain positive communication with residents, families, and community professionals regarding the Brookdale dementia care program and its advantages, offering updates on residents’ individual needs and achievements.
  • Assist the Manager or Coordinator in achieving departmental quality improvement initiatives, including utilizing the Brookdale Excellence Standards Tool (BEST).
  • Facilitate program activities outside the community, which may involve operating a community vehicle.
  • Support the volunteer program by training and supervising volunteers.
  • Perform personal care, housekeeping, and dietary responsibilities as needed to uphold service quality in accordance with Clare Bridge Program standards.

Qualifications:

Education and Experience:

  • A High School Diploma or equivalent is required; an Associate’s Degree is preferred.
  • A minimum of one year of experience working with older adults and individuals with dementia is preferred, or a comparable combination of education and experience.

Certifications, Licenses, and Other Special Requirements:

  • A valid driver’s license and a current three-year driving abstract with no more than one moving violation are required conditions of employment.

Physical Demands and Working Conditions:

  • The role requires standing, walking, sitting, using hands and fingers, reaching, climbing, stooping, kneeling, and the ability to lift up to 50 pounds.
  • Duties involve interaction with residents, co-workers, and vendors, and may require occasional weekend, evening, or night shifts to ensure coverage.
  • Potential exposure to communicable diseases, latex, and varied hazards may be present, with occasional travel required.

Knowledge and Skills:
