In alignment with Brookdale’s Resident and Family Engagement philosophy, the Clare Bridge Program Coordinator is responsible for planning, coordinating, and facilitating a calendar of resident-centered, dementia-friendly programs that cater to the specific needs of all residents within the dementia care community. This role enhances residents’ quality of life by providing meaningful and purposeful engagement, honoring individual abilities and preferences while promoting independence through tailored care and programming.
Key Responsibilities:
– Design and implement a 7-day weekly program that is engaging, creative, and reflective of individual residents’ needs and interests.
– Develop a monthly calendar in partnership with residents, ensuring that special events and daily programs are effectively scheduled and executed.
– Ensure compliance of dementia care programs with Brookdale Excellence Standards Tool (BEST), state and federal regulations, and internal policies.
– Collaborate with community leadership to educate families and associates about Alzheimer’s disease and other dementias, and facilitate relevant training programs.
– Provide direct supervision of the Clare Bridge Program Assistant(s) and manage performance evaluations as directed by the Executive Director.
– Plan and coordinate special events, including holidays and community outreach initiatives.
– Communicate significant changes in residents’ health to clinical partners and address behavioral concerns collaboratively.
– Oversee social media content to ensure alignment with Brookdale’s branding and standards.
– Organize external programming events, which may involve driving a community vehicle.
– Manage departmental budgets and may be involved with the volunteer program as needed.
– Perform personal care, housekeeping, and dietary duties to maintain service quality as per Clare Bridge Program standards.
Qualifications:
– Bachelor’s Degree in therapeutic recreation, gerontology, education, or a related field preferred.
– A minimum of two years of direct programming experience with older adults and individuals with dementia, or an equivalent combination of education and experience.
Certifications, Licenses, and Special Requirements:
– A valid driver’s license along with a recent driving record indicating no more than one moving violation is required. A commercial driver’s license may be required based on community needs.
Physical Demands and Working Conditions:
– Prolonged periods of standing, walking, and sitting, as well as the ability to utilize hands for tasks such as handling and reaching.
– Ability to lift items up to 50 pounds and engage in physical activities associated with resident care.
– Interaction with co-workers, residents, and vendors is essential, with the potential for weekend or evening shifts as required.
– Possible exposure to health hazards, including communicable diseases and chemicals.
