In alignment with Brookdale’s Resident and Family Engagement philosophy, the Clare Bridge Program Coordinator is responsible for planning, coordinating, and facilitating a calendar of person-centered programs tailored to meet the specific needs of residents in our dementia care community. This role focuses on providing meaningful and purposeful activities that enhance the quality of life for our residents while recognizing their individual abilities and preferences.

Key Responsibilities:

  • Design and implement a weekly program that is engaging and meets the individual interests of residents, promoting well-being through various structured activities, including sensory-focused small group programs for those with participation limitations.
  • Collaborate with residents to develop a monthly calendar reflecting their interests, ensuring that special events and daily programs are scheduled and executed effectively.
  • Ensure compliance with Brookdale’s Excellence Standards Tool (BEST), as well as state and federal regulations, to uphold Brookdale’s mission and dementia care standards.
  • Educate and provide resources to families and associates about Alzheimer’s disease and dementia, including leading training sessions and coordinating in-services.
  • Provide direct supervision to Clare Bridge Program Assistants, including assisting with hiring, performance evaluations, coaching, and documenting corrective actions as necessary.
  • Collaborate with community leadership to plan and participate in special events, ensuring they are appropriately decorated and marketed.
  • Report any changes in residents’ health and behavior to clinical partners in a timely manner, collaborating on potential interventions.
  • Oversee social media content to ensure alignment with Brookdale’s brand and values.
  • Schedule and coordinate programming events outside the community, which may involve driving community vehicles.
  • Adhere to the department’s budget and expense control measures, and may manage the volunteer program.
  • Perform personal care, housekeeping, and dietary responsibilities as needed, in accordance with Clare Bridge Program standards.

Qualifications:

Education and Experience: A Bachelor’s Degree in therapeutic recreation, gerontology, education, or a related field is preferred. A minimum of two years of direct programming experience with older adults and individuals with dementia is also preferred, or an equivalent combination of education and experience.

Certifications, Licenses, and Other Special Requirements: A valid driver’s license and a current 3-year driving record showing no more than one moving violation are required; a commercial driver’s license may be necessary depending on community needs.

Physical Demands and Working Conditions:

  • Requires standing, walking, sitting, reaching, climbing, and the ability to lift up to 50 pounds.
  • Involves interaction with residents, co-workers, and vendors, and may include occasional evening or weekend shifts.
