In line with Brookdale’s Resident and Family Engagement philosophy, the Clare Bridge Program Coordinator is responsible for planning, coordinating, and facilitating a calendar of person-centered, dementia-friendly programs tailored to meet the specific needs of all residents within our dementia care community. This role emphasizes person-centered care, engaging residents in meaningful programs that enhance their quality of life. The Coordinator will recognize individual capabilities while fostering independence, ensuring that residents’ habits, historical routines, interests, and preferences are honored in all programming.
Key Responsibilities:
- Design and implement a comprehensive 7-day-a-week program that is engaging and tailored to enhance each resident’s interests.
- Develop a monthly program calendar in collaboration with residents, ensuring that special events and daily activities are appropriately scheduled and executed.
- Ensure that all dementia care programs comply with Brookdale Excellence Standards Tool (BEST), as well as relevant state and federal regulations, and align with Brookdale’s mission and policies.
- Collaborate with community leadership to educate and provide resources for families and associates about Alzheimer’s disease and other dementias, including facilitating training and in-services as required.
- Supervise Clare Bridge Program Assistants as needed, including responsibilities related to hiring, coaching, counseling, and performance evaluations.
- Plan and coordinate special community events, ensuring appropriate decorations and participation.
- Report health changes of residents to the clinical partner and address concerns promptly, collaborating with the team on behavioral interventions as necessary.
- Oversee social media content to ensure it aligns with Brookdale’s brand and standards.
- Organize and schedule external programming events, which may include operating community vehicles.
- Manage departmental budget and may oversee a volunteer program.
- Perform personal care, housekeeping, and dietary responsibilities as needed to uphold service quality in accordance with Clare Bridge Program standards.

Qualifications:
- A Bachelor’s Degree in therapeutic recreation, gerontology, education, or a related field is preferred.
- A minimum of two years of direct programming experience with older adults and individuals with dementia is preferred, or an equivalent combination of education and experience.
Certifications and Requirements:
- A valid driver’s license and a current 3-year driving record with no more than one moving violation are required for employment, with the potential need for a commercial driver’s license based on community requirements.
Physical Demands and Working Conditions:
- This position requires various physical activities, including standing, walking, sitting, and capability to lift up to 50 pounds.
- Interaction with co-workers, residents, and vendors is essential, as is occasional work during weekends, evenings, or nights.
