Base pay within the established range will be determined by the candidate’s skills and experience, and the role is eligible for team-based bonus opportunities. Please note that temporary associates are not eligible for benefits but may participate in the company’s 401(k) program.

Responsibilities

Job Summary
In line with Brookdale’s Resident and Family Engagement philosophy, the Clare Bridge Program Coordinator will plan, coordinate, and facilitate a calendar of person-centered, dementia-friendly programs tailored to meet the specific needs of all residents within the dementia care community. The Coordinator will focus on delivering engaging, meaningful, and purposeful dementia care programs that enhance the quality of life for residents while respecting and honoring their individual abilities, interests, and historical routines.

Essential Functions

  • Design and implement a weekly schedule of engaging, creative programs that cater to the unique needs and interests of residents, encompassing the seven domains of well-being, while providing sensory-focused small group activities for residents who may not participate in larger gatherings.
  • Collaborate with residents to develop a monthly calendar reflecting shared interests, ensuring that special events and daily programs are scheduled and executed as planned.
  • Ensure compliance of dementia care programs with Brookdale’s Excellence Standards Tool (BEST), divisional dementia care site visit requirements, and applicable state and federal regulations while upholding Brookdale’s mission and care standards.
  • Partner with community leadership to educate and provide resources to families and associates regarding Alzheimer’s disease and other dementias, while facilitating ongoing training and in-services related to dementia care.
  • Supervise Clare Bridge Program Assistants when necessary, including assistance with hiring, coaching, counseling, performance evaluations, and documentation of corrective actions.
  • Collaborate with community leadership to plan, coordinate, and actively participate in special events, including holiday celebrations, signature programs, and educational events.
  • Monitor and report any changes in residents’ health and behavior to the clinical team promptly, collaborating with them on appropriate interventions.
  • Oversee social media content in alignment with Brookdale’s culture, brand standards, and social media guidelines.
  • Organize and schedule external programming events, which may involve operating community vehicles.
  • Maintain and adhere to departmental budgetary guidelines and may supervise volunteer programs.
  • Perform personal care, housekeeping, and dietary responsibilities as needed to uphold service quality in accordance with Clare Bridge Program standards, having completed the necessary training to ensure compliance with state regulations.

Qualifications

Education and Experience
A Bachelor’s Degree in Therapeutic Recreation, Gerontology, Education, or a related field is preferred, along with a minimum of two years of direct programming experience with older adults and individuals with dementia, or an equivalent combination of education and experience.

Certifications, Licenses, and Other Special Requirements
Candidates must hold a valid driver’s license and maintain a current three-year driving record that shows no more than one moving violation as a condition of employment. Depending on community needs, a commercial driver’s license may also be required.
