Position Summary:
In alignment with our organization’s mission to enhance the lives of the residents we serve, the Charge Nurse plays a pivotal role in providing exceptional customer service and nursing care to all residents within the assigned unit, as well as throughout the facility. The Charge Nurse collaborates with the Director of Nursing, Unit Managers, and/or the Assistant Director of Nursing to plan, develop, and manage the overall operations of the assigned unit, ensuring residents receive high-quality care 24 hours a day.

Principal Duties and Responsibilities:

  • Assess residents’ conditions to implement interventions and develop Care Plans, ensuring compliance with medical records policies and procedures.
  • Conduct regular rounds both independently and with a physician; observe and evaluate residents’ symptoms, progress, and responses to treatments and medications, implementing necessary interventions.
  • Provide preventative, supportive, maintenance, and rehabilitative care aimed at addressing the physiological and psychosocial needs of residents.
  • Administer treatments to residents, including but not limited to changing dressings, cleansing and irrigating wounds, Foley catheter irrigations, IV therapy, tube feedings, and peritoneal dialysis.
  • Coordinate and ensure that necessary supplies and medical equipment are available for resident care and treatments.
  • Schedule appointments with physicians, therapy sessions, and necessary consultations.
  • Monitor and support CNAs/STNAs in delivering personal care to residents, including bathing, feeding, and assisting with ambulation, among other routine services.
  • Receive reports from outgoing Charge Nurses and perform daily rounds with all residents as needed.
  • Administer medications to residents following established health codes, nursing department policies, and physician prescriptions; ensure timely communication regarding any medication contraindications.
  • Keep residents and/or their responsible parties informed about changes in medications, treatment plans, and overall conditions.
  • Manage and document emergency care for residents, facilitating communication with relevant parties including physicians, family members, and emergency services.
  • Conduct comprehensive and focused nursing assessments in response to changes in residents’ conditions.
  • Assist with the admission, readmission, and transfer of residents within the assigned unit, ensuring the availability of necessary supplies and equipment.
  • Oversee the completion of weekly and discharge summaries.
  • Participate in quality improvement initiatives as directed by the Director of Nursing, Unit Manager, or ADON.
  • Monitor the frequency of accidents, incidents, and infections within the unit, proposing corrective actions as necessary.
  • Report any resident or staff complaints, along with unusual occurrences, to the Director of Nursing or Administrator, and escalate any allegations of abuse or neglect immediately.
  • Contribute to periodic regulatory surveys conducted by the State of Michigan and assist with the preparation of correction plans in response to inspection findings.
  • File reports on resident injuries to the Unit Manager.
  • Support the facility’s philosophy, standards, policies, and procedures.
  • Communicate with department heads as necessary to enhance patient care effectiveness.
  • Alert the Director of Nursing or Unit Manager/ADON regarding unmet staffing requirements.
  • Present a professional appearance and serve as a role model within the unit.
  • Maintain professional development by engaging in work-related seminars and continuing education opportunities.
  • Promote adherence to safety protocols and accident prevention measures, contributing to the welfare of both employees and residents.
  • Ensure safe usage of equipment and report maintenance concerns to the Director of Maintenance, along with any hazardous conditions or supply issues.
  • Adhere to the Nurse Practice Act, consistently maintaining resident comfort, privacy, and dignity, while ensuring a warm, respectful interaction.
  • Respond promptly to call lights in resident care areas, ensuring courteous service.
  • Communicate and interact effectively with residents, their families, visitors, peers, and supervisors.
  • Attend staff meetings and in-service training as requested, fulfilling continuing education requirements necessary for professional licensure.
  • Understand and implement infection control guidelines, including handwashing principles, isolation procedures, and infection recognition.
  • Uphold and protect resident rights by facilitating informed decision-making, ensuring dignity and respect, safeguarding personal belongings, and avoiding unnecessary physical restraints.
  • Undertake other tasks as required.

Supervisory Responsibility:
This position provides general supervision to Certified Nursing Assistants (CNAs) and State Tested Nursing Assistants (STNAs), including retraining and counseling sessions, and implementing disciplinary actions as needed. The Charge Nurse does not supervise Registered Nurses.

Requirements:

Education, Training, and Experience:

  • Must possess a current state license as a Registered Nurse or Licensed Practical Nurse.
  • CPR certification required.
  • One year of professional nursing experience in a long-term care setting is preferred but not mandatory.
  • Knowledge of applicable state nursing regulations is advantageous.

Specific Skills, Knowledge, and Abilities:

  • Familiarity with current federal and state laws and regulations pertinent to nursing practice in a long-term care environment.
  • Strong organizational and time management skills, with the ability to prioritize responsibilities effectively.
  • Proficiency in applying mathematical concepts such as fractions, percentages, ratios, and proportions in practical scenarios.
  • Basic computer literacy is required.
