We are currently seeking a Home Health Physical Therapist Assistant (15-20 hours) to become a valued member of our team at AdvisaCare. In this position, you will collaborate closely with clients under the supervision of a licensed Physical Therapist. Your key responsibilities will include assisting in the implementation of tailored treatment plans, facilitating therapeutic exercises, and providing education on appropriate techniques for mobility and rehabilitation. Furthermore, you will engage with interdisciplinary healthcare teams to guarantee seamless care coordination and promote optimal patient outcomes.

Responsibilities:
- Assist in the implementation of individualized treatment plans developed by the licensed Physical Therapist.
- Conduct therapeutic exercises and activities with clients to enhance their mobility, strength, and coordination.
- Provide guidance and education to clients and their families on proper techniques for exercises, transfers, and ambulation.
- Assess and document clients’ progress, modifying treatment plans as necessary.
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes.
- Communicate effectively with clients, families, and caregivers, providing support and addressing questions.
- Adhere to all applicable legal, ethical, and professional standards.
Requirements:
- Degree from an accredited Physical Therapy Assistant program.
- Current licensure as a Physical Therapist Assistant in the state of Michigan.
- CPR certification.
- Strong interpersonal and communication skills.
- Ability to work independently as well as part of a team.
- Flexibility to adapt to changing client needs and schedules.
