Job description:

Allegria Village is seeking a Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team.

Education/Certification:

  • Graduate of an accredited Bachelor’s, Master’s, or Doctorate in Physical Therapy.
  • Current license issued by the state Board of Physical Therapy Examiners.

Job Responsibilities:

  1. Deliver direct patient care, assess functional needs and outcomes, and collaborate with other specialists. Evaluate patients’ functional requirements and modify care plans to ensure optimal outcomes.
  2. Develop and implement a comprehensive plan of care for patients based on thorough assessments and encourage participation from significant others when appropriate.
  3. Conduct therapy practices in alignment with established standards of care.
  4. Supervise the support staff to help achieve patients’ functional goals effectively.
  5. Communicate proficiently with other healthcare professionals and support staff to enhance patient outcomes.
  6. Adhere to facility and departmental policies, procedures, objectives, and quality improvement programs, as well as safety, environmental, and infection control standards.
  7. Document evaluations, treatment goals, and plans accurately; regularly update documentation and maintain necessary prescriptions and signatures.
  8. Promote professional development for oneself and others through participation in educational programs, in-service meetings, and workshops; remain informed about current professional literature.
  9. Foster positive working relationships with other healthcare professionals and ancillary staff.
  10. Perform additional duties as assigned.

Qualifications:

  • Comprehensive knowledge and skills to meet the care needs of the Skilled Nursing Facility population.
