Job description:
Under the supervision of the Children’s Special Health Care Services/Child Lead, Nurse Family Partnership, or Maternal Infant Health Program Supervisor, the Public Health Nurse (PHN) plays a critical role in addressing community health needs by defining, planning, implementing, and evaluating public health nursing services primarily targeted at families with incomes below the threshold for WIC. The PHN delivers comprehensive and professional nursing assessments and services to children, pregnant individuals, families, and the broader community through programs such as Children’s Special Health Care Services, Child Lead, Maternal Infant Health Program, or Nurse Family Partnership. Additionally, the PHN may assist in specific initiatives like the Fetal Infant Mortality Review (FIMR) or Child Death Review (CDR).
Essential Functions
- Conduct home, school, office, and community visits to evaluate the physical, emotional, social, and environmental needs of individuals and families, focusing on healthy development and life course health.
- Establish rapport with clients while fostering their autonomy, self-esteem, and problem-solving capabilities.
- Enhance community health literacy by creating evidence-based educational materials and conducting group teaching sessions on topics such as lead exposure reduction, childbirth, and car seat safety.
- Collaborate interprofessionally with all members of the client’s care circle.
- Assist clients in identifying and establishing specific, measurable, achievable, relevant, and timely health and wellness goals.
- Develop and implement personalized care plans to meet targeted health goals, including reducing lead exposure, parenting support, postpartum care, nutrition, child healthcare, family planning, and special health needs.
- Monitor progress toward goals and continually review and adjust care plans as necessary.
- Facilitate appropriate referrals to community agencies and resources based on client needs.
- Serve a diverse clientele, including families who primarily speak languages other than English.
- Participate in Continuous Quality Improvement initiatives as mandated by specific programs.
- Engage in provider education and outreach activities.
- Utilize data to inform interventions aimed at achieving optimal health outcomes for clients.
- Collaborate with State of Michigan personnel to ensure compliance with program requirements and effective service delivery.
- Adhere to best practices in nursing documentation and record all nursing activities promptly.
- Pursue professional development opportunities and continuing education to enhance nursing practice.
- Conduct case extraction services for FIMR and/or CDR, which may include reviewing medical records with providers, interviewing community services involved with families, or interviewing families who have experienced infant or child loss.
- Present findings from case extractions to FIMR and/or CDR committees for review and to inform recommendations for the prevention of infant and child deaths.
- Contribute to FIMR and/or CDR committees in formulating recommendations for death prevention.
- Engage in public policy education and advocacy, as guided by program directives.
- Collaborate professionally with various community organizations, such as Local Leadership Groups or Great Start Coalitions.
- Participate in various committees within the designated program or Ingham County to advocate for positive organizational growth and change.
- Educate nursing students in public health nursing practices, including opportunities for shadowing home visits or participating in group education sessions led by the PHN.
- Ensure that client records are maintained securely and confidentially, adhering to Health Department policies and HIPAA standards.
- In the event of a public health emergency, perform duties that may extend beyond those outlined in this job description.
Employment Qualifications
- Education: Possession of a Bachelor’s Degree in Nursing.
- Licensure: A current, unrestricted license to practice as a Registered Nurse in the State of Michigan.
- Experience: A minimum of two years of nursing experience.
Desired Qualifications:
- Possession of a vehicle and current auto insurance or reliable means of travel throughout Ingham County.
- Valid driver’s license in the State of Michigan, or equivalent.
- Valid BLS certification. Demonstrates effective communication skills that engage, interact, and work with individuals from varying cultural, socioeconomic backgrounds, and diverse age, disability, LGBTQIA+, and gender identities to ensure clarity and understanding in all interactions.

Physical/Technological Requirements:
- Ability to access office files and retrieve information from a computer.

- Working Conditions:
