Public Health Project Coordinator – Data to Care
Under the supervision of the HIV Care Manager, the Public Health Project Coordinator – Data to Care will play a pivotal role in the implementation of the Data to Care model within the City of Detroit. This position involves the development and maintenance of all applicable Data to Care policies and procedures. The coordinator will spearhead community outreach and educational initiatives, oversee the Senior Community Services Assistant – HIV/STI and Community Services Assistant, and engage in investigations aimed at reconnecting out-of-care individuals to health services. Collaboration with the HIV Surveillance Team is crucial to monitor client progress and adjust programs as necessary. Additionally, the coordinator will conduct quality assurance activities, lead quality improvement projects, and document process evaluations and outcomes.
Examples of Duties:
– Collaborate with the Michigan Department of Health and Human Services to manage the Not-In-Care list effectively.
– Design and maintain summary reports to facilitate compliance with state reporting requirements and support the development and evaluation of quality improvement projects. Assist in the preparation of grant reports and applications.
– Coordinate data-related activities with partner agencies, including the Michigan Department of Health and Human Services epidemiologists and CAREWare staff.
– Identify relevant data sources and establish and maintain relationships with associated entities.
– Present findings and reports to stakeholders, including planning council members and Ryan White providers, through written, electronic, and verbal means of communication.
– Participate in Public Health Emergency Preparedness activities during actual or simulated emergencies.
– Undertake additional duties as assigned by the supervisor.
Minimum Qualifications:
– Master’s Degree in Public Health or a related field.
– A minimum of three (3) years of experience in data management and statistical analysis.
– Proven experience working with individuals living with HIV.
