Lighthouse MI is a dynamic non-profit organization established in 2019 through the merger of Lighthouse of Oakland County and South Oakland Shelter, aimed at enhancing the collective response to poverty in the Metro Detroit community. With over 80 years of combined experience, Lighthouse serves as a beacon of hope, providing assistance with emergency basic needs, housing programs, and supportive services that promote long-term economic stability. Lighthouse also owns and operates Spero Housing Group, a dedicated subsidiary for developing and managing high-quality affordable and supportive housing for low- and moderate-income individuals, alongside HandUp, a community crowdfunding platform that supports over 100 homeless service agencies and their clients nationwide.
Position Summary
The Case Manager is responsible for managing a caseload of 20 to 30 families, assessing individual and family needs, developing tailored action plans, assisting with budgeting, and monitoring compliance with program requirements. The role involves conducting intakes and exits, documenting client interactions in the Homeless Management Information System (HMIS), and ensuring timely case notes and service transactions are completed to accurately measure program outcomes.
Key responsibilities include:
- Supporting clients in finding, securing, and maintaining housing.
- Conducting unit Housing Quality Standards (HQS) inspections to ensure compliance with safety and federal regulations.
- Providing ongoing case management for housed families at least once a month.
- Utilizing the Mainstream Benefits Access Tool to screen clients for and connect them to appropriate benefits and resources.
- Implementing and enforcing program rules, policies, and procedures to foster a safe environment.
- Maintaining accurate client files and records.
- Administering client assessments and evaluations, and making referrals to additional services when needed.
- Engaging with landlords for mediation, negotiation, and tenant relations as necessary.
Additionally, the Case Manager will:
- Maintain authorization for client transportation and provide these services as required.
- Foster inter-agency collaborations to facilitate client connections to shelter and support long-term stability.
- Continuously enhance case management and data collection practices.
- Participate in front-desk operations, answering phones and managing crisis calls when needed.
- Join internal and external task forces and committees focused on aiding the homeless population.
Minimum Qualifications
- Lived or professional experience working with individuals facing homelessness or extreme poverty in the U.S.
- An associate degree.
- 1-2 years of experience in roles related to human/social services or case management.
- Reliable transportation, including maintenance of automobile insurance, and a valid driver’s license.
- Proficiency in Microsoft Office Suite and Google Apps.
- Flexibility and the ability to prioritize tasks in a fast-paced, dynamic environment.
- Strong multitasking abilities while managing diverse populations.
- Availability to work evenings and weekends.
Preferred Qualifications
- A Bachelor of Social Work (BSW) from an accredited institution or a master’s degree in a related field.
- 2+ years of experience in human/social services or case management roles.
- A comprehensive understanding of social constructs affecting economically unstable populations.
- Knowledge of the social services landscape in Oakland County and surrounding areas.
- Experience with HMIS.
- Capacity for self-regulation and independent work while collaborating with team members.
- Enthusiasm for innovation and creative problem-solving.
- Training in Harm Reduction, Trauma-Informed Care, Housing First, and SPDAT methods.
- Proven success in engaging with racially and ethnically diverse clients and teams.
- Experience in team management and leadership.
- Strong communication skills, both written and oral, with the ability to accept feedback constructively and demonstrate high emotional intelligence.
