The role of the Resident Services Coordinator is central to facilitating a supportive atmosphere for our senior residents by providing educational, vocational, and other opportunities that foster self-improvement and the realization of personal goals. The RSC will build and maintain partnerships with community-based social service providers and various stakeholders, enabling residents to effectively meet their needs through targeted educational programs and service access.

Responsibilities include assessing individual residents’ requirements for social service intervention based on referrals from property management or self-referrals. The RSC will coordinate with non-profit social service providers and, when appropriate, monitor residents’ outcomes, documenting all engagements and referrals in detailed case notes. Establishing robust community relationships is crucial for helping residents navigate and manage their social service needs effectively.

Essential Functions:

  • Develop strategic partnerships with local service providers to deliver on-site programs or facilitate referrals to off-site services, focusing on outcomes in education, health and wellness, workforce development, financial literacy, transportation, nutrition, personal care, and asset-building.
  • Provide non-clinical case management and ensure residents access the full range of required social services.
  • Initiate and maintain collaborative relationships with local service providers and community stakeholders.
  • Engage residents and their families to better understand their needs and connect them with relevant local resources and opportunities.
  • Serve as a liaison between residents and Property Management, community agencies, and regional government entities such as the Social Security Administration and the Department of Veterans Affairs.
  • Collaborate with Property Management staff, eldercare service providers, and medical professionals to address issues while respecting confidentiality.
  • Monitor and assist residents in managing cognitive, emotional, and physical health challenges, including Activities of Daily Living (ADL) limitations, and coordinate moves to care facilities when necessary, including transportation arrangements.
  • Maintain proactive outreach and communication with residents through newsletters, flyers, phone calls, and home visits.
  • Ensure accurate and timely documentation of resident demographics, issues, requests, incidents, interactions, and service outcomes using software systems aligned with Vesta’s mission and goals.
  • Participate in and coordinate resources for critical issues such as housekeeping and financial management for residents with late rent payments.
  • Handle administrative tasks related to securing and executing contracts with service providers, such as Memorandums of Understanding.
  • Develop and maintain connections with area resources, ensuring access to social and recreational services that meet community and individual needs.
  • Perform additional duties and responsibilities as assigned.

Knowledge, Skills, and Abilities:

  • Demonstrated ability to respect and respond to diverse cultural values, attitudes, and beliefs in program planning and implementation.
  • Proficiency in building and maintaining positive relationships with a range of stakeholders, including residents, staff, volunteers, interns, and community partners.
  • Experience in working with seniors and individuals with disabilities to support their aspirations and needs.
  • Knowledge of human services/social services, including familiarity with the aging services network and eldercare systems.

Requirements

Required Education and Experience:

  • A bachelor’s degree focused on social work, human services, public health, psychology, developmental disabilities, community development, or a related field is preferred.
  • A minimum of 2 years of professional experience in housing or human services agencies is preferred.
  • Experience working with seniors is highly valued.
  • Experience in overseeing and managing grants is a plus.
  • Proficiency in MS Excel, PowerPoint, Word, Outlook, and other software is essential.
  • Prior experience in property management, along with knowledge of HUD regulations, is advantageous.
  • Experience collaborating with residents from diverse socioeconomic and cultural backgrounds is preferred.
  • Ability to work with partners and site team members in effectively implementing programs and initiatives.
  • A valid driver’s license and a clean driving record are required.
  • Successful completion of a background check and drug screening is mandatory.
