Job description:
Become an integral part of our compassionate community and help us prioritize health as a Home Health Aide (HHA).
The Home Health Aide will:
- Deliver direct patient care under the guidance of a Registered Nurse (RN) and according to the Aide Plan of Care (POC).
- Assist patients with self-administered medications by opening containers, reading labels, verifying prescribed dosages, and ensuring adherence to medication schedules.
- Accurately measure and record vital signs such as temperature, pulse, and blood pressure, while promptly reporting abnormal findings to the supervisor.
- Support patients in maintaining personal hygiene through activities including bathing, grooming, skin care, shaving, oral care, and nail/foot care, ensuring dignity and comfort.
- Assist with feeding patients, demonstrating basic nutritional principles, observing and documenting food and fluid intake as required, while safely positioning patients during meals.
- Facilitate patient toileting needs, including the use of a bedpan or urinal, managing catheter bags, and performing ostomy care as necessary.
- Apply skills to assist patients with mobility, exercises, positioning, transfers, and ambulation, in accordance with the POC and CenterWell Home Health policies.
- Document patient interactions, care provided, and any changes in condition accurately and in a timely manner to ensure continuity of care.
- Implement appropriate infection control practices to maintain a clean, safe, and welcoming environment.
- Assist with light household duties such as laundry, bed making, meal preparation, and shopping, in accordance with any existing medical orders when no other assistance is available.

Required Experience/Skills:
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Compliance with all applicable state certification requirements.
- Possession of a valid driver’s license, auto insurance, and reliable transportation.
- Good standing on the HHA Registry (if applicable) and completion of HHA/CNA training to be eligible for Medicare-certified agencies.
- Minimum of one year of experience in the past 24 months as a Home Health Aide or Certified Nursing Assistant in settings such as hospitals, nursing homes, or home health/hospice agencies.
