As a Home Health Aide (HHA), your responsibilities will include:

– Delivering direct patient care under the guidance of a registered nurse, following the established Aide Plan of Care (POC).
– Assisting patients with self-administered medications by opening bottle caps, reading medication labels, verifying the prescribed dosage, and ensuring that medications are taken as directed.
– Accurately measuring and recording vital signs—such as temperature, pulse, and blood pressure—while promptly reporting any abnormal findings to the supervising nurse.
– Supporting patients in maintaining personal hygiene through bathing, grooming, skin care, shaving, oral care, nail and foot care, and other related activities.
– Aiding in the feeding of patients; communicating basic nutritional principles, observing and documenting food and fluid intake as needed, and safely positioning patients for meals to assist in self-feeding.
– Assisting with toileting needs, including the use of bedpans and urinals, as well as managing catheter bags and ostomy care.
– Utilizing appropriate techniques for patient mobility support, including exercises, positioning, turning, transferring, and ambulation as outlined in the Plan of Care and CenterWell Home Health policies.
– Documenting changes in patient conditions and care provided, ensuring continuity of care through accurate and timely reporting of interactions with patients, caregivers, and healthcare professionals.
– Practicing established infection control principles and ensuring a clean, safe, and comfortable environment for patients.
– Willingly assisting with light household duties such as laundry, bed changing, light meal preparation, housekeeping, and shopping when no other assistance is available and under a physician’s order.

Required Experience/Skills:

– High school diploma or equivalent.
– Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
– Compliance with applicable state certification standards.
– Possession of a valid driver’s license, auto insurance, and reliable transportation.
– Good standing on the HHA Registry (if applicable) and successful completion of HHA/CNA course for eligibility to work with a Medicare-certified agency.
– A minimum of one year of experience in a home health, hospice, hospital, or nursing home setting within the last 24 months.
