The Shelter Association of Washtenaw County (SAWC) is seeking a dedicated Veteran and Assessment Case Manager to join our team. This position reports to the Assessment Team Manager and Program Director. Established in 1982, SAWC is the leading provider of emergency shelter and support services for individuals experiencing homelessness in Washtenaw County, where an estimated 5,000 individuals face homelessness. Located at the Robert J. Delonis Center in downtown Ann Arbor, we cater to a variety of populations, including first-time homeless individuals, veterans, survivors of domestic violence, and those struggling with substance abuse and mental health challenges. Our core programs encompass Residential Emergency Shelter, Non-Residential Support, Shelter Diversion, Critical-Time Intervention, and Warming Shelter, collectively serving nearly 1,500 individuals annually.

Position Overview:
The Veteran and Assessment Case Manager will orient 20 or more clients to the shelter weekly by conducting single adult coordinated entry service assessments while managing a caseload of approximately eight veteran clients. This role operates within a collaborative team comprised of another Assessment Case Manager and two seasonal winter intake specialists, under the supervision of the Assessment Team Manager. The primary objective is to offer a warm and supportive entry experience for clients and guide them toward community resources and additional support services. Many clients will be awaiting the availability of a residential bed; thus, completing assessments will allow access to essential shelter services, placement on the Community Housing Prioritization list, linking to housing case management, and opportunities to apply for housing vouchers. The position also entails providing supportive services to veteran clients in collaboration with the Healthcare for Homeless Veterans (HCHV) program. The work schedule is primarily weekdays from Monday to Friday, with occasional evening and weekend shifts.

Key Responsibilities:

General Responsibilities:

  • Ensure the smooth operation of the Delonis Center.
  • Provide transportation for clients using a personal vehicle when needed.
  • Conduct client meetings in the community as required.
  • Maintain detailed and up-to-date records in the HMIS system to ensure case files are complete.
  • Provide support to other case managers as necessary.
  • Work evenings, weekends, and holidays during winter months as assigned.
  • Undertake additional assignments as directed by the Manager.

Assessment Case Manager Responsibilities:

  • Conduct 20 or more coordinated entry service assessments weekly for single adults experiencing homelessness.
  • Effectively connect clients with appropriate community resources and make referrals to relevant SAWC services.
  • Guide clients in accessing shelter services, including securing a housing case manager and/or a Housing Choice Voucher.
  • Maintain and follow up on a triage list for clients in need of residential beds.
  • Complete Housing Choice Voucher pre-applications and homeless verification letters as necessary.
  • Connect clients with SAWC’s Client Resource and Diversion Specialist when diversion from shelter is a suitable option.
  • Assist with front desk coverage and community meal monitoring when required.
  • Actively engage in case review team meetings and community housing coordination discussions.

Veteran Case Management Responsibilities:

  • Conduct initial assessments and intake for veteran clients entering shelter from residential and winter programs.
  • Assist veteran clients with accessing supportive services and housing case management.
  • Manage referrals for veterans seeking Community Emergency Residential Services (CERs) at the Delonis Center.
  • Ensure a welcoming orientation and accommodating experience for veterans utilizing the CER beds.
  • Use SQUAREs to assess veteran client eligibility for services.
  • Refer veteran clients to SSVF housing options when they meet the criteria.
  • Complete the necessary documentation for the Veteran Administration promptly.
  • Update client information using real-time data entry in the HMIS, including detailed case notes for veteran clients.
  • Attend community level Veteran Community Housing Prioritization meetings.
  • Collaborate with the Veteran Outreach Case Manager, Peer Support Specialist, and Contracting Officer to address veteran client needs and individual service plans.

Qualifications:

  • A Bachelor’s degree in Social Work or a related field is required; a Master’s in Social Work or a similar degree is preferred. Candidates with LLMSW status must agree to obtain LMSW status by an agreed-upon date. Equivalent experience may substitute for formal degrees.
  • 1-2 years of experience in health care or human services organizations is preferred.
  • Strong diagnostic skills, particularly in mental health and substance abuse areas.
  • Experience with HMIS or similar electronic records systems is strongly preferred.
  • A comprehensive understanding of the health care system and community resources.
  • Excellent verbal and written communication skills.
  • Strong critical thinking abilities.
